St. Joseph's Home Health Care extends the reach of skilled services from
hospitals, clinics, and physician offices into the home. Working under
the direction of your physician, our services are available 24 hours a
day, 7 days a week.
St. Joseph's Home Health Care services can help to improve the quality of life for our patients and offer convenience for their families and physicians. Home health care can help patients to live as fully as possible at home with their loved ones.
Our family-centered approach includes teaching patients and their families about their care, coordinating additional services, and researching community resources. Our billing specialist will also verify home care payments with private insurances, Medicare, Medicaid, private-pay and third-party payers.
Who is eligible for home health care?
Patients of all ages who live in Onondaga County are eligible for our services. You may be awaiting discharge from an area hospital or already at home needing additional health care support.
Do I need a doctor's order to obtain services?
Yes. A physician's order is
required to begin services, but we will be glad to call your physician
for you or a loved one.
When are services available?
A St. Joseph's home care nurse will do an in-home evaluation within 24 hours of receiving a physician's order. We provide services for patients 24 hours a day, seven days a week.
How do I pay for home health care services?
Our agency services are covered by many private insurance companies, Medicare, Medicaid and private pay. Our office staff will help you to understand your insurance coverage and to obtain other means of financial assistance, if needed. We offer a payment scale based on your ability to pay.
