With diabetes, what you don't know can hurt you. Take charge of your diabetes. Diabetes is a serious health problem. If you have diabetes, you are at increased risk for heart disease, kidney disease, loss of vision, or amputation. That's the bad news. The good news is that if you take charge of your diabetes, you can greatly reduce your risk of serious complications.

St. Joseph's Diabetes Self-Management Education Program will teach you what you need to know to keep your diabetes under control. You'll get the support you need to make lifestyle changes from our team of nurses, dietitians, counselors, and exercise specialists. You'll meet other people just like you who are learning to manage their diabetes at home, at work and at play.

St. Joseph's Take Charge will help you put your doctor's treatment plan into action with specific guidelines for checking your blood sugar and balancing diet, medication and exercise. Our qualified staff will work closely with you and your physician.

How Take Charge works:

  • The course lasts approximately six weeks.

  • The first two sessions are private

  • Group sessions will provide education, hands-on practice of techniques, group discussion and support.

  • Each week you will be given a packet of materials for home use.

  • Six week after the last meeting you will return for a follow-up visit.



All meetings are at the St. Joseph's Health Care Network Building on the North Medical Campus located on Janus Park Drive. Our building is accessible to individuals with disabilities, and we have free parking.
